Requirements include:

  • Doctor of pharmacy (Pharm.D.) from an accredited school of pharmacy acceptable to the Surgeon General
  • License to practice pharmacy
  • Between 21 and 42 years of age (may request a waiver)
  • Permanent U.S. residency
